Oakridge Dental Member Sign-up

1. Complete and submit form the above. This will create a Peelz Scrubs account and connect it to the Oakridge Dental Peelz Partnerz account.
2. You’ll get an email when you submit the form. As a Oakridge Dental member, your application will be automatically approved and store credit will be loaded into your account.
3. Log in to your new Peelz Scrubs account. (You’ll get a passcode sent to your email, so there’s no need to set or change a password.)
4. Shop like normal. You’ll see your discounted price on the product pages.
5. Ensure you use store credit at checkout.

If you have a Shop account, you’ll need to log out of your Shop account and log in with your Peelz Scrubs account.

You can use your existing account. If you use the same information as when you created your Peelz Scrubs account, then your existing account with be connected to the Oakridge Dental Peelz Partnerz account.
